To participate in this study, you need to be at least 18 years old and have had a kidney transplant for two or more years. Unfortunately, if you received a different type of transplant (like a liver or heart transplant) or if you have difficulty measuring your blood pressure or blood sugar, you would not be eligible. Participants will receive training on how to use the app, and the study is currently looking for volunteers. This trial aims to make a positive difference in the care of kidney transplant patients and help them achieve better health outcomes.

- Inclusion Criteria:
- • ≥18 years of age
- • ≥2 years post-kidney transplant
- Exclusion Criteria:
- • Non-kidney transplant recipient (liver, lung, heart, intestine, pancreas, bone marrow)
- • Not capable of measuring own BP and glucose in those with diabetes
- • Not capable of using mobile health application after adequate training
- • Not capable of speaking, hearing, and reading English
