This clinical trial is studying how a special type of ultrasound, called transthoracic ultrasound, can help predict if patients will have difficulty breathing after being taken off a ventilator in the respiratory intensive care unit (ICU). The researchers want to see if certain measurements from the ultrasound, like how well the diaphragm (the muscle that helps us breathe) is moving, can indicate whether a patient might struggle after extubation, which is when the breathing tube is removed.
To be eligible for this trial, participants must be at least 18 years old, admitted to the respiratory ICU, and on a ventilator due to lung issues but have successfully completed weaning trials (tests to see if they can breathe on their own). However, those who are pregnant, have certain muscle or nerve disorders, psychiatric instability, serious heart problems, or have had a tracheostomy (a surgical opening in the neck for breathing) will not be included. If you join this study, you can expect to undergo a transthoracic ultrasound, which is a painless and non-invasive procedure, to help doctors gather important information about your breathing ability. The trial is not yet recruiting participants, so more information will be available soon.

- Inclusion Criteria:
- • Patients above 18 yrs. old who admitted to respiratory ICU and mechanically ventilated due to chest diseases and passed weaning trials
- Exclusion Criteria:
- • Pregnancy.
- • Neuromuscular disorders.
- • Psychiatric instability.
- • Uncontrolled cardiovascular disease.
- • Tracheostomy.
- • patints with failed weaning trials.
- • Inability to provide informed consent.
