This clinical trial is studying a new way to monitor how hard patients are working to breathe while on mechanical ventilation in the Intensive Care Unit (ICU). When patients struggle to breathe on their own, doctors sometimes use a method called "diaphragmatic protection" to help them gradually regain their strength. This trial aims to improve how doctors measure the effort patients use to breathe by using a non-invasive technique called Occlusion Pressure. By considering both how hard patients breathe and how long they breathe for, the researchers hope to provide better assistance through the ventilator.
To be eligible for this trial, participants must be adults aged 65 and older who are on mechanical ventilation and have had difficulty with at least three attempts to breathe on their own. They also need to have a special device called an esophageal balloon placed to help with measurements and must be stable in their overall health. Participants can expect to take part in tests that will help validate this new measurement method, which could improve future care for patients needing help with breathing. It's important to note that individuals under 18, pregnant women, or those with poor health outlooks are not eligible to participate.

- Inclusion Criteria:
- • Patients with mechanical ventilation, who failed 3 or more spontaneous breathing trials
- • Esophageal balloon placed
- • Who can trigger the ventilator
- • With hemodynamic stability
- • with P/F ratio above 150
- Exclusion Criteria:
- • under 18 years old of age
- • pregnancy
- • do not resuscitate order or expected poor short term prognosis
- • refuse to sign the informed consent
