This clinical trial is studying two different treatments for carpal tunnel syndrome (CTS), a condition that can cause pain, numbness, and weakness in the hand. The researchers want to compare the effectiveness of an injection of dextrose (a type of sugar solution) given with ultrasound guidance to an injection that combines dextrose with a steroid called methylprednisolone. The goal is to find out which treatment helps reduce pain and improve hand function better over a six-month period.
To participate in the trial, you need to be between 18 and 80 years old and have been diagnosed with idiopathic carpal tunnel syndrome, which means the cause of your CTS is unknown. However, there are some reasons you might not be able to join, such as being allergic to the treatments, having had recent wrist surgery or injuries, or certain health conditions like diabetes or thyroid disease. If you do take part, you’ll receive one of the treatments and will be monitored for changes in pain levels, grip strength, and overall quality of life during and after the study. This trial is currently looking for volunteers, and your participation could help improve future treatments for CTS.

Eligibility criteria
- Inclusion Criteria:
- • age between 18 and 80 years,
- • the diagnosis of idiopathic CTS.
- Exclusion Criteria:
- • contraindications for local application of methylprednisolone or 5% dextrose (hypersensitivity to the drug, infection and skin damage at the application site),
- • previous wrist surgery,
- • traumatic wrist injury within 2 years,
- • previous wrist injection within 6 months,
- • previous physical therapy within 6 months,
- • history of peripheral traumatic nerve injury in the upper extremity, brachial plexopathy, severe cervical radiculopathy, and thoracic outlet syndrome,
- • history of diabetes
- • history of thyroid disease,
- • history of inflammatory rheumatic disease,
- • pregnancy,
- • inability to cooperate with the study protocol.
