This clinical trial is studying the effectiveness of a Virtual Community of Practice (VCoP) designed to help middle-aged individuals (ages 30-60) who have two or more chronic diseases, also known as multimorbidity. The aim is to see if participating in an engaging online community can better activate and empower these patients compared to traditional self-guided online education. Participants will be randomly assigned to either join the VCoP for 12 months, which includes social interaction and gamified elements, or to receive individual online education without social support.
To be eligible, participants must be middle-aged and have at least two chronic conditions. Those who may have difficulties with cooperation, are in transitional housing, or have severe health issues that prevent them from completing surveys will not be included. Throughout the study, participants will complete surveys at different times to measure their engagement and well-being. This trial will also evaluate the cost-effectiveness of the VCoP, which means it will look at both the benefits and expenses related to this approach compared to standard education. Eligibility criteria
- Inclusion Criteria:
- • Middleagedpeople (30-60 years old)
- • with multimorbidity (≥=2 chronic diseases).
- Exclusion Criteria:
- • Low probability of cooperation in the study
- • situation of transients or displaced persons, institutionalized, with some terminal illness, physical or mental disability that prevents them from answering the questionnaires correctly.
