The MIVacuna trial is testing whether adding motivational interviewing (a respectful, collaborative counseling approach to talk through vaccine concerns) to behavioral health visits can reduce vaccine hesitancy and increase uptake of COVID-19 and flu vaccines among Latinx adults with mental illness. The study uses a real-world design across clinics in Boston, comparing a new approach (with an electronic vaccine prompt, motivational interviewing during visits, and an option to hand you to a nurse for vaccination) to standard care. About 6,000 adults are being invited to participate, and the study runs through 2027.
If you might qualify, you would be an adult (18 or older) who identifies as Latinx, has recently had a behavioral health visit within the study period, and is not up-to-date with either the COVID-19 vaccine or the flu vaccine at the start of that visit. If eligible, you would first see an alert in the electronic health record about missing vaccines, then your clinician would discuss vaccination using motivational interviewing; if you choose, you could be directly referred to a nurse to receive the vaccine during the same visit. The main goal is to see whether people end up getting vaccinated within about six weeks after the visit, with extra surveys about vaccine hesitancy, trust in the provider, and vaccination intentions. The trial is taking place at two Boston clinics and results will come after the study finishes.

Eligibility criteria
- Patient Inclusion Criteria:
- • Aged 18 years and older
- • Identifies as Latinx
- • Completed an eligible behavioral health (BH) visit between the start and end dates of the intervention period.
- • Patient is missing a COVID-19 vaccine/booster or the influenza vaccine at the beginning of their behavioral health visit (i.e., they are not fully up to date with their COVID-19 and influenza vaccines)
