This clinical trial is studying a new testing method called shotgun metagenomics to see how well it can diagnose chronic prosthetic joint infections (PJI). This type of infection can occur in patients who have had joint replacement surgery. The researchers want to find out if this new method is better at identifying the infections compared to traditional culture tests, which is how bacteria are usually grown and identified in a lab. They will also look at the types of bacteria causing these infections and whether they are resistant to antibiotics.
To participate in the study, individuals must be 18 years or older and are being considered for surgery due to suspected chronic PJI, meaning their infection has been present for more than three months after their joint replacement. Participants will undergo routine care, which includes taking six samples during their surgery and an extra blood sample to measure inflammatory markers. This study is currently recruiting participants and aims to include 143 patients from various centers in Western France. It's important to know that those with certain health conditions, like active blood infections or those who are pregnant, cannot take part in this trial.

Eligibility criteria
- Inclusion Criteria:
- • Male or female ≥ 18 years
- • Patients being considered for surgery for suspected chronic PJI (time between joint replacement surgery and patient inclusion \> 3 months)
- • Social security affiliation
- • No objection to participating in the study
- Exclusion Criteria:
- • Suspicion or documentation (positive blood cultures) of acute bacteremia at time of inclusion
- • Patients under guardianship or trusteeship
- • Pregnant or breastfeeding woman
- • Patient deprived of liberty by legal or administrative decision
- • Patients in psychiatric care
