This clinical trial is looking at a procedure called thoracoscopic pleural lavage and brushing, which doctors use to help diagnose lung-related issues in patients who have fluid build-up in the space around their lungs, known as pleural effusion. Sometimes, doctors can't determine the exact cause of this fluid even after performing initial tests. This study aims to see how effective and safe this procedure is for those patients who need further investigation.
To be eligible for this trial, patients should have a specific type of pleural effusion that hasn't been clearly diagnosed after initial tests, along with solid clinical evidence suggesting there may be another underlying issue. However, certain patients won't be able to participate, such as those with specific breathing difficulties or bleeding conditions. Participants can expect to undergo a procedure where doctors will collect samples from the pleura (the lining around the lungs) to help find out what might be causing their symptoms. It's important to note that the trial is not yet recruiting participants, so no one can sign up just yet.

Eligibility criteria
- Inclusion Criteria:
- • Patients will have documented exudative pleural effusion in whom the initial pleural tapping and closed pleural biopsy (CPB) will not be conclusive
- • strong clinical and radiological data suggestive for alternative pathological diagnosis
- • who will be admitted to our department
- Exclusion Criteria:
- • Patients with excess rib crowding with narrow inter-costal space and loculated pleural effusion cannot undergo thoracoscopy
- • Patients with bleeding diathesis
- • hemodynamic instability
- • Arrhythmias
- • intractable cough cannot be eligible to do thoracoscopy
