The GO TEAM trial is studying how to help Black/African American children and young adults with type 1 diabetes use diabetes management technologies, like continuous glucose monitors and insulin pumps, more effectively. These technologies can help manage blood sugar levels, but many in this group are not using them as much as others. The researchers want to see if a new computer program for healthcare teams, a smartphone app called Glucosano for patients and families, and support from community health workers can make a difference in how these tools are accepted and used.
To join the trial, participants need to be Black and have type 1 diabetes that requires insulin. They should be willing to try a continuous glucose monitor for 10 days at three different times. It's important to note that only those who haven’t used an automated insulin delivery system in the last three months can participate. This study aims to ensure that everyone, regardless of race, has equal access to important diabetes management tools, helping them lead healthier lives.

Eligibility criteria
- Inclusion Criteria:
- • Self-reported as Non-Hispanic Black
- • Clinical diagnosis of T1D requiring treatment with insulin at the time of consent
- • Not currently (within the past 3 months) using an automated insulin delivery system \\
- • Willingness to wear a continuous glucose monitor for 10 days at 3 different time points, and willingness to consider use of a personal continuous glucose monitor
- Exclusion Criteria:
- • Clinical diagnosis of Type 2 or monogenic diabetes
- • Completed high school
- • Non-English speaking guardians
- • Automated insulin delivery system use within the past 3 months
- • Custody of children and family services
