This clinical trial is looking to compare two types of pain relief methods used during heart valve replacement surgeries: intrathecal dexmedetomidine and intrathecal morphine. Both medications are given through a small injection into the spinal area to help manage pain after surgery. The goal is to see which option works better for relieving pain and supporting breathing after the operation.
To be eligible for this trial, patients must be scheduled for elective open heart valve replacement surgery and be between the ages of 65 to 74. However, certain patients may not qualify, such as those with bleeding disorders, allergies to the study drugs, or other serious health conditions like chronic lung diseases or previous surgeries that could complicate the procedure. If someone participates in this study, they will be monitored closely to see how well the chosen pain relief method works for them. It’s important to note that this trial is not yet recruiting participants, but it aims to help improve pain management for future heart surgery patients.

Eligibility criteria
- Inclusion Criteria:
- • - Cardiac patient scheduled for elective open heart valve replacement surgery.
- Exclusion Criteria:
- • • Patient refusal
- • Coagulation disorders
- • History of known allergy to the used drugs.
- • Combined procedures (e.gif combined with coronary artery bypass or aortic root surgeries).
- • If thoracotomy or min-sternotomy is planned for the surgery.
- • Re-do and emergency surgeries.
- • History of chronic chest diseases (COPD or IPF).
- • History of thoracotomy, pneumothorax, pneumomediastinum, phrenic nerve injury (as evident by the presence of paralysis of the ipsilateral hemidiaphragm when examined preoperatively).
- • Neuromuscular diseases.
- • Brain injuries.
