This clinical trial, called the TabCAT Brain Health Assessment in Primary Care, is looking at a new way to identify cognitive problems, such as mild cognitive impairment and dementia, in patients. You should also have had at least one visit to one of the participating clinics during the study period. If you join the study, you can expect to use the tablet assessment during your visit, which will help your doctor understand your brain health better. This trial is currently recruiting participants, and everyone is welcome, regardless of gender.

- • Patient with at least one encounter at an intervention clinic during the intervention period
- • Age 18+ (limited to 65+ for primary analyses)
- Exclusion Criteria:
- • Patients under the age of 18.
