This clinical trial is looking at how a medication called dexmedetomidine may help prevent delirium, which is a sudden change in mental status, in elderly patients who are having emergency surgery called laparotomy. Delirium is especially common in older adults after surgery, and the goal of this study is to see if dexmedetomidine can reduce the chances of developing this condition.

To participate in this trial, patients need to be 60 years old or older and must be scheduled for an emergency laparotomy, whether due to an injury or another health issue. However, some people may not be eligible, such as those who cannot give consent, are younger than 60, or have certain health conditions like severe liver disease or major cognitive disorders. If eligible, participants will receive the study medication during their hospital stay and will be monitored for any effects on their mental status. This trial is currently not yet recruiting, but it could provide valuable information about improving care for elderly patients undergoing surgery.

Eligibility criteria

  • Inclusion Criteria:
  • • Patients aged 60 years or older of either gender.
  • Patient undergoing emergency laparotomy either trauma or non-trauma patients.
  • Exclusion Criteria:
  • • Patients unable to provide written consent.
  • Patients less than 60 years.
  • Known drug intolerance or allergy to dexmedetomidine.
  • Patients previously diagnosed to suffer from major neurocognitive disorder, defined by a mini-mental state examination (MMSE) score \< 24.
  • Severe audiovisual impairment.
  • Child C chronic liver disease.
  • Traumatic brain injury.
  • Intracranial bleeding \< 1y.
  • Psychiatric illness.
  • History of alcohol or drug abuse.
  • Preoperative cerebrovascular accident with residual neurological deficit.
