The Eye Health Intervention Study in Upper Manhattan (EYES-UM) aims to improve eye care for adults aged 40 and older who may not have easy access to eye health services. This study will provide eye screenings to people living in Upper Manhattan, focusing on those who are at risk for conditions like glaucoma, diabetic retinopathy, cataracts, and other vision problems. By partnering with local primary care offices and senior centers, the study plans to reach around 17,000 individuals who may not have had regular eye check-ups. If participants do not pass the eye screening, they will receive immediate care from an eye doctor and will get free eyeglasses if needed.
To be eligible for this study, participants must be at least 40 years old and able to live independently. Unfortunately, those with a serious illness that limits their life expectancy or who cannot understand the study's requirements due to conditions like dementia will not be able to participate. Throughout the study, which will last for two years, participants will receive support in scheduling follow-up eye exams and transportation if needed. This trial will not only help improve eye health in the community but will also evaluate the costs and benefits of the care provided compared to standard services.

- • Self-reported terminal illness with life expectancy of less than 1 year.
- • Inability to provide informed consent due to dementia or other reason.
