This clinical trial is investigating a new method to manage pain after total knee replacement surgery (TKA) for patients with severe arthritis. The study will compare a special type of injection that combines different pain-relieving medications (called multimodal drug injection) with the usual pain management treatments like opioids and anti-inflammatory drugs. The goal is to see if the injections can reduce pain more effectively and help patients recover better, while also checking if there are any additional risks or complications from using this new method.

To participate in this study, patients must be adults aged 18 to 75 who are planning to have a total knee replacement due to end-stage arthritis. However, those with certain conditions, such as inflammatory arthritis, allergies to the injection medications, or a history of opioid addiction, cannot join. Participants will receive either the new injection or standard pain relief and will be monitored for their pain levels and recovery progress. This trial is an opportunity for patients to potentially benefit from improved pain management techniques following their surgery.

Eligibility criteria

  • Inclusion Criteria:
  • all patients with end-stage arthritis of the knee going primary unilateral total knee arthroplasty.
  • Exclusion Criteria:
  • inflammatory and secondary arthritis of the knee.
  • Patients who are allergic to one or more of the drugs used in the injections.
  • Patients who are already taking opioids for whatever reason or have a history of addiction.
  • BMI less than 20 and more than 35.
  • patients with intra-operative complications that would affect the outcomes measurement.
  • Patients who are going through complex primary or revision TKA.
  • Pregnancy, renal or liver failure.
  • Patients who are classified as grade 3 or more according to ASA.
