This study from Sydney is asking how to make the move from pediatric (child) care to adult diabetes care smoother for people with type 1 diabetes. It is an observational cohort, meaning researchers will watch and learn from what happens, not test a new treatment. They plan to enroll about 80 participants at Royal North Shore Hospital and Mona Vale Community Health Centre, including both those who have already started or completed the transition and those who will begin it. The main focus is on areas that matter to everyday life—how confident people feel about managing diabetes, what they know about their condition, and how much distress or worry diabetes causes them. They will also look at health results like blood sugar control (HbA1c), emergency department visits, and any diabetes-related complications over up to five years, with periodic reviews.

Who can take part and what to expect: people eligible are 16 years or older, have type 1 diabetes, and are transitioning from pediatric to adult care at the two Sydney sites (starting from 2020–2023 or from June 2023 onward). A family member or caregiver and a staff member from the diabetes team can participate as part of the study. Participants will complete online questionnaires (accessible via a QR code) and health information will be collected to follow outcomes over time. This is not a trial of a treatment, and individual data will not be shared outside the study.

Eligibility criteria

  • Inclusion Criteria:
  • Adolescents who have an existing diagnosis of type 1 diabetes mellitus and who underwent (in 2020 to 8th June, 2023) or who are undergoing (from 9th June, 2023 and subsequent cohorts) transition from paediatric care to adult health care at Royal North Shore Hospital and Mona Vale Community Health Centre. Adolescents with type 1 diabetes will be 16 years or older
  • A family member or caregiver of the participant of the adolescent patient with type 1 diabetes mellitus
  • A staff member who is a member of the patient's treating diabetes team within the paediatric diabetes service (first study visit and second study visit) and young adult diabetes service (third study visit)
  • Exclusion Criteria:
