The Active Transition Home Post-Stroke Program is a clinical trial designed to help stroke survivors maintain their activity levels as they transition from rehabilitation back to home. Participants will wear special sensors that track their daily activities, sleep patterns, and how long they sit still. Only those in the intervention group will receive personalized feedback based on this data and have sessions with a physiotherapist to set achievable activity goals. This study aims to see how useful these sensors and goal-setting sessions are in supporting stroke survivors during this challenging time.
To be eligible for this trial, participants must be at least 18 years old and have experienced a stroke that has affected one side of their body. They should also be able to speak English or have someone who can help with communication. The study will involve interviews three months after discharge to gather participants' thoughts on their experiences with the sensors. This trial is not yet recruiting participants, but it will provide valuable insights into how technology can assist stroke survivors in staying active at home.

Eligibility criteria
- Inclusion Criteria:
- • Physiotherapists for focus group participation
- • Age of 18 or older
- • Registered Physiotherapist at GF Strong (inpatient or outpatient)
- • 1 year of work experience in stroke rehabilitation (acute or chronic)
- • Patient Group
- • Age of 18 or older
- • Had a stroke resulting in hemiparesis
- • English speaking or living with an English-speaking family member to assist with interpretation/goal setting with PT
- Exclusion Criteria:
- Physiotherapist Focus Group:
- • - \<1 year of experience in stroke rehabilitation
- Patient Group:
- • - presents with any other unstable medical conditions that would impact activity (e.g., unstable cardiac condition, recent fracture)
