This clinical trial is studying a tool called the Patient-Oriented Discharge Summary (PODS), which aims to make the transition from hospital to home easier for patients, especially those with multiple health conditions. When patients leave the hospital, they often feel overwhelmed and may forget important information about their care. The PODS is a simple one-page summary that includes key details such as why the patient was hospitalized, warning signs to watch for, treatment plans, and future medical appointments. The goal of this study is to see if using the PODS can help improve the quality of care and reduce problems for patients after they leave the hospital.
To participate in this trial, you need to be at least 18 years old and have been hospitalized for more than 48 hours. You also need to be able to understand and communicate in French and be able to give your consent to join the study. If you join, you can expect to work with the healthcare team to complete the PODS before you leave the hospital. This study is currently recruiting participants, and your involvement could help improve the discharge process for future patients.

Eligibility criteria
- Inclusion criteria:
- • Being discharged home
- • Able to speak, read and write in French
- • Being hospitalized for more than 48H in participating medical units
- • Being able to give informed consent as documented by signature
- Exclusion criteria:
- • • Inability to follow the procedures of the study according to the health care team, due to language problems or cognitive impairment.
