This clinical trial is studying the effects of a medication called aminophylline when added to another drug, dexmedetomidine, during surgery. The goal is to see how this combination can improve lung function and oxygen levels in patients who have chronic obstructive pulmonary disease (COPD) and are morbidly obese, specifically those undergoing laparoscopic bariatric surgery, which is a type of weight-loss surgery done with small incisions.
To be eligible for this trial, participants should be between 20 and 60 years old, have a body mass index (BMI) over 40, and have been diagnosed with COPD. They should also not have had any previous abdominal surgeries and be in good psychological health. Participants will receive the study medications during their surgery and will be closely monitored for changes in their lung function and oxygen levels. It's important to note that individuals who refuse to participate, have certain heart or other serious health issues, or have allergies to the study medications will not be included in the trial.

Eligibility criteria
- Inclusion Criteria:
- • Age from 20 to 60 years.
- • Patients scheduled for laparoscopic bariatric surgery not exceeding three hours under general anesthesia.
- • American Society of Anesthesiologists (ASA) class II to III.
- • Patients who had no previous abdominal surgery.
- • Patients with a body mass index (BMI) greater than 40 kg/m2and suffered obstructive lung disease diagnosed by pulmonary function tests (; forced vital capacity (FEV1\\FVC) \< 70%).
- • Patients who are psychologically normal.
- Exclusion Criteria:
- • Patients who refuse to participate.
- • Those with arrhythmia or receiving anti-arrhythmic medications.
- • Those with significant coexisting diseases (e.g. kidney, heart, liver).
- • Those with known allergy to any of allocated drugs.
- • Those with excessive intraoperative blood loss more than allowable amount to every patient.
