This clinical trial is studying two different methods of draining fluid from the chest in patients with empyema, which is an infection in the pleural space around the lungs. The researchers want to see if using a technique called thoracoscopy, which involves inserting a small camera into the chest to help with drainage, leads to better results compared to the traditional method of using a tube for drainage alone. They will compare how long patients stay in the hospital, whether they need any additional procedures, and how often the treatment fails. To participate in this trial, you must be 18 years or older and have a confirmed diagnosis of empyema, shown through imaging tests like a chest X-ray or CT scan. You should have symptoms such as fever, chest pain, or cough, and there should be specific fluid collections that need to be drained. If you've had any previous drainage procedures for the current empyema or have certain health conditions that could complicate the procedure, you may not be eligible. This trial is not yet recruiting participants, but it aims to provide important information that could improve the treatment of empyema in the future.

Eligibility criteria
- Inclusion Criteria:
- • Confirmed diagnosis of empyema via Imaging tests (e.g. chest x-ray/CT scan)
- • Age 18 years or older
- • Clinical signs/symptoms consistent with Empyema such as fever, chest pain, coughs
- • Pleural fluid loculations/septations seen on Imaging requiring drainage
- • No previous drainage procedures done for Current empyema
- Exclusion Criteria:
- • Age less than 18 years
- • Immunocompromised state or other conditions Contraindications to thoracoscopy
- • Previous drainage procedure for current Empyema
- • Residual pleural fluid not amenable to drain
