The Exacerbantes Study is a clinical trial aimed at understanding how different factors affect patients with acute worsening of chronic obstructive pulmonary disease (COPD). COPD can cause breathing difficulties, and during an exacerbation, these symptoms can get much worse. Researchers want to identify specific traits in patients that can help tailor treatment more effectively. This study will look at 400 patients who experience a sudden increase in respiratory symptoms, both in primary care settings and hospital emergency departments.
To be eligible, participants need to be 40 years or older, have a history of smoking, and show signs of breathing problems. Those with certain serious conditions, like recent cancer or specific chest issues, won't be included. Participants will undergo routine tests, such as chest X-rays and blood tests, to gather important information about their health. After 90 days, the team will check back in to see how patients are doing. This study aims to improve how we manage COPD exacerbations by understanding the individual differences among patients.

Eligibility criteria
- Inclusion Criteria:
- • Men or women who request emergency care (primary care or hospital) due to acute worsening of respiratory symptoms (of any nature)
- • Age 40 years or older.
- • Smokers or ex-smokers of \> 10 pack-years
- * Expiratory airflow limitation, documented by any of the following methods:
- • Post bronchodilation FEV1/FVC \< 0.7, in a previous forced spirometry, or
- • FEV1/FEV6 less than 0.73 on microspirometry performed during decompensation
- Exclusion Criteria:
- • Patients with active neoplasia in the last 3 years.
- • Associated pleural effusion susceptible to thoracentesis.
- • Pneumothorax.
- • Thoracic trauma.
- • Impossibility of subsequent follow-up (3 months).
