This clinical trial is investigating a surgical procedure called lateral cervical node dissection in patients with differentiated thyroid cancer that has spread to the lymph nodes in the neck. The goal is to compare two different surgical approaches—one from the front and one from the back of a specific neck muscle—to see which method causes less discomfort and is more effective in removing cancerous nodes.
To be eligible for this trial, participants must be at least 18 years old and have signs of cancer in the lymph nodes in their neck, either through physical exams or imaging tests. They should also have a confirmed diagnosis of a specific type of thyroid cancer. People with certain previous neck surgeries or specific types of thyroid cancer are not eligible. If you join the study, you can expect to have surgery and be monitored for how well you recover and how effectively the cancer is treated. This trial aims to improve treatment options for thyroid cancer patients and help doctors understand the best surgical techniques.

Eligibility criteria
- Inclusion Criteria:
- • 1. Patients ≥ 18 years.
- • 2. Patients with macroscopic lymph node involvement identified by physical examination, imaging or intraoperatively in lateral neck.
- • 3. Patients with microscopic nodal involvement confirmed by FNAB (definition by the pathologist of suspected or confirmed metastatic papillary carcinoma according to the Bethesda criteria)
- • 4. Candidates for lateral lymph node dissection due to suspected or confirmed disease metastatic lymph nodes as defined by the treating surgeon.
- • 5. Patients requiring or not requiring thyroidectomy and/or central dissection concomitant with the dissection
- Exclusion Criteria:
- • 1. Patients with a history of previous neck dissection
- • 2. Histological confirmation of medullary or anaplastic carcinoma
- • 3. Previous spinal nerve injury
