This clinical trial is studying a new program to help people who smoke tobacco quit. The program is designed to be delivered by pharmacists at Federally Qualified Health Centers (FQHCs), which are healthcare facilities that primarily serve low-income patients. The goal is to provide support for smokers who are ready to quit and to offer additional help for those who aren't quite ready yet, encouraging them to reduce their smoking first. The researchers want to see if this program is practical and effective in these community settings and to identify any challenges that might come up in implementing it.
To participate in the trial, you need to be at least 18 years old, a patient at a participating FQHC, and currently smoke at least 5 cigarettes a day. You should also be able to read, speak, and understand English, own a cell phone, and be willing to use nicotine replacement therapy (like patches or lozenges) to help with quitting. It's important to note that if you’ve had certain health issues recently, like a heart attack or stroke, you may not be eligible to join. If you qualify, you can expect support from pharmacists and a structured plan to help you on your journey to quitting smoking.

Eligibility criteria
- Inclusion Criteria:
- • patient at participating Federally Qualified Health Center
- • ≥18 years of age
- • able to read, speak, and understand English
- • report smoking ≥5 cigarettes per day for the past 6 months
- • own a cell phone
- • be willing and able to use NRT in the form of patch or lozenge
- • not be pregnant or planning to be pregnant in the next 6 months
- Exclusion Criteria:
- • have a medical contraindication to NRT (e.g., past 30 days, heart attack or stroke; past 6 months, serious or worsening angina, very rapid or irregular heartbeat requiring medication)
