This clinical trial is looking at how special consultations with trained nurses can help people with diabetes who are using insulin injections or insulin pumps. The goal is to see if these consultations can improve blood sugar control, quality of life, and reduce complications related to diabetes. It’s particularly aimed at helping those who may be struggling to manage their diabetes effectively.
To participate in this study, you need to be between 18 and 75 years old and either have type 1 or type 2 diabetes that requires insulin. You should also be using a specific glucose monitoring system called FreeStyle Libre, and your blood sugar levels need to be out of balance (with a test called HBA1c over 8). Unfortunately, if you have recently been diagnosed with diabetes, have a significant intellectual disability, or are pregnant, you won't be eligible to join. If you do participate, you can expect to have extra support and education from the nurses, which could help you better manage your diabetes in your daily life.

- Inclusion Criteria:
- • Woman or man aged 18 to 75
- • Diabetic patient type 1 or 2 insulin-dependent
- • Patient treated with multi-injections or insulin pump
- • Patient equipped with FreeStyle Libre flash glucose monitoring system
- • Glycemic imbalance with HBA1c \> 8
- • Patient living at home
- Exclusion Criteria:
- • Recent discovery (\< 6 months) of diabetes
- • Major intellectual disability
- • Social isolation
- • IMC \> 35
- • Pregnant women
