This clinical trial is investigating whether using a personalized approach to a specific breathing support technique called high PEEP (positive end-expiratory pressure) can reduce the risk of lung complications after bariatric surgery. Bariatric surgery is a type of weight-loss surgery that can help people with obesity, but it can also lead to breathing problems after surgery, especially when general anesthesia is used. The trial aims to compare the effects of this personalized high PEEP method with a standard PEEP technique to see which one better supports patients’ lungs during recovery.
To participate in the study, individuals need to be between 18 and 60 years old, have a body mass index (BMI) over 40, and be scheduled for elective laparoscopic bariatric surgery under general anesthesia. Participants will be monitored during and after their surgery to assess how well their lungs are functioning. It's important to note that people with certain health conditions, like severe lung disease or those who have had lung surgery before, will not be eligible for this trial. This study is currently recruiting participants, and it offers a chance to potentially improve recovery outcomes for patients undergoing this type of surgery.

Eligibility criteria
- Inclusion Criteria:
- • Patients were eligible for participation if the met the following criteria
- • Age between 18 and 60 years old.
- • Body mass index \> 40
- • Laparoscopic bariatric elective surgery under general anesthesia.
- • ARISCAT (Assess Respiratory Risk in Surgical Patients in Catalonia) score ≥ 45 . ARISCAT score is a clinically practical seven-factor scoring system to assess the risk of a composite PPC.
- Exclusion Criteria:
- • Previous lung surgery
- • ASA status III or IV.
- • Moderate to severe obstructive or restrictive lung disease .
- • Persistant intraoperative hemodynamic instability .
- • Need for postoperative mechanical ventilation
