This clinical trial is studying the use of a medication called Tolvaptan in patients who have a condition known as hyponatremia, which means they have low sodium levels in their blood. Specifically, the trial is looking at the safety and effectiveness of giving a lower-than-usual dose of Tolvaptan to patients who have moderate to severe hyponatremia linked to a syndrome called SIADH, which causes the body to retain too much water. The researchers want to see if this lower dose can help those patients, especially since some may only need a single dose while others might require one or two doses.

To participate in the trial, patients need to be between 18 and 90 years old and have been admitted to the hospital with low sodium levels that haven't improved with standard treatments. However, pregnant women and anyone under 18 cannot take part. Participants will receive the treatment as part of their usual hospital care and will be monitored for how well the medication works and any side effects. This study is important because it could provide valuable information about using a safer, lower dose of Tolvaptan, which may help more patients with this condition.

Eligibility criteria

  • Inclusion Criteria:
  • All patients presenting to Accident \& Emergency department or seen as inpatients with moderate to severe hyponatraemia (sNa: \<125mmol) associated with SIADH and not responding to conservative measures of hyponatraemia management.
  • Patients of all genders aged 18-90 y.o
  • Exclusion Criteria:
  • Pregnant women and patients aged \< 18y.o
  • Hyponatraemia not associated with SIADH
