ClinConnect Summary

This clinical trial, called the Cardio-share Telemedicine Cross-sector Collaboration Model, is looking at how a new telemedicine approach can help manage patients with heart failure. Heart failure is a condition where the heart doesn't pump blood as well as it should, which can lead to hospital admissions. The study aims to understand what makes some patients more vulnerable to these health issues and to identify key elements that can help manage their care effectively using telemedicine tools. By comparing the number of hospital readmissions and patients' quality of life before and after implementing this model, researchers hope to find ways to improve care for those who frequently need hospital treatment.

To join the study, participants must have been admitted to the hospital for heart failure and either had a previous admission for the same issue in the last three months or at least two admissions in the past six months. Participants will receive support to use telemedicine tools, and their doctors will have access to video calls with heart specialists. Community health workers will also assist patients at home. This collaborative approach aims to provide better care and support for patients with heart failure, helping them manage their condition more effectively.

Eligibility criteria

  • Inclusion Criteria:
  • Patients admitted with acute decompensated heart failure AND:
  • with previous admission with decompensated heart failure in the three-month period prior to the current admission or
  • who have two or more admissions in the 6 months prior to the current admission.
  • Exclusion Criteria:
  • Patients who
  • have refused to share their data for health-quality projects
  • have already ongoing telemedicine management for heart failure
