This clinical trial is studying how two commonly used anesthetics, desflurane and sevoflurane, may affect the risk of delirium in older patients after surgery. Delirium is a serious condition that can cause confusion and changes in awareness, and it is more likely to happen in patients aged 65 and older. The goal of the study is to see if the type of anesthetic used during surgery can influence whether patients experience delirium afterward.
To participate in this trial, patients need to be between 65 and 90 years old and scheduled for a surgery that lasts at least two hours, but it should not involve the heart or brain. They must also be able to understand and agree to take part in the study by signing a consent form. Participants will receive one of the two anesthetics during their surgery, and researchers will monitor their recovery to see how well they do afterward. It's important to note that patients with certain medical conditions or communication difficulties may not be eligible for this study.

Eligibility criteria
- Inclusion Criteria:
- • 1. Age ≥ 65 years and \< 90 years;
- • 2. Scheduled to undergo non-cardiac or non-neurosurgery with an expected duration of 2 hours or more, under general anesthesia;
- • 3. Agree to participate, and give signed written informed consent.
- Exclusion Criteria:
- • 1. Preoperative history of schizophrenia, epilepsy, parkinsonism or any diseases of central nervous system;
- • 2. Inability to communicate in the preoperative period (coma, dementia, language barrier, impaired hearing or vision);
- • 3. Severe diseases in cardiovascular, respiratory, liver, kidney, or preoperative American Society of Anesthesiologists physical status classification ≥ IV;
- • 4. Alcoholism and drug dependence;
- • 5. Other reasons that are considered unsuitable for participation by the responsible surgeons or investigators (reasons must be recorded in the case report form).
