This clinical trial is looking at whether a type of treatment called cognitive intervention therapy can help prevent confusion, known as postoperative delirium, in older patients after non-heart-related surgeries. Specifically, the study will focus on patients aged 65 and older who are having orthopedic, thoracic, or liver surgeries that last at least two hours. The trial will compare the effects of this therapy, which involves mental exercises and support, to regular treatment without this therapy.
To be part of this study, patients must be at least 65 years old and scheduled for a qualifying surgery. However, those with serious health issues like uncontrolled diabetes or high blood pressure, certain vision or communication difficulties, cognitive impairments, or neurological disorders are not eligible to participate. The trial is not yet recruiting participants, but it's an important step in understanding how to better support older patients during their recovery from surgery. Participants can expect to receive either cognitive intervention therapy or standard care as part of the study.

Eligibility criteria
- Inclusion Criteria:
- • Elderly patients aged 65 and above.
- • Patients undergoing orthopedic, thoracic, or hepatopancreatobiliary surgery under general anesthesia with an expected surgery duration of 2 hours or more.
- • Patients scheduled for arterial catheterization.
- Exclusion Criteria:
- • Patients with uncontrolled systemic conditions such as diabetes and hypertension.
- • Those with visual impairment.
- • Patients with cognitive impairment based on the MMSE-DS criteria.
- • Individuals experiencing difficulty in communication.
- • Those diagnosed with neurological disorders (such as brain hemorrhage, stroke, brain tumor, dementia, Parkinson's disease, cognitive impairment, depression, etc.).
- • Patients diagnosed with alcohol or substance addiction.
