This clinical trial is studying different methods to assess how well patients with low blood pressure (hypotension) respond to fluid treatments. The focus is on patients who may have difficulty thinking clearly or are less aware of their surroundings. The researchers want to find out which techniques are most accurate in predicting how these patients will respond to receiving fluids, which is important for their recovery.
To be eligible for this study, participants need to be at least 18 years old, currently hospitalized in an intensive care unit, and have a certain level of impaired consciousness and low blood pressure. They should also have a specific type of medical device (a central venous catheter) in place. However, some patients may not qualify due to conditions like serious lung issues or significant kidney problems. Those who participate will help doctors understand which methods work best for keeping patients stable and improving their health when they have both low blood pressure and cognitive challenges.

Eligibility criteria
- Inclusion Criteria:
- • Age ≥ 18 years.
- • Hospitalization in an intensive care unit
- • Level of consciousness on the FOUR scale ≤ 12 points
- • Presence of arterial hypotension
- • Presence of a central venous catheter or direct indication for catheterization
- • The decision of the medical council to include the patient in the study
- Exclusion Criteria:
- • Pulmonary edema
- • Absence of one or both lower extremities
- • Chronic kidney disease according to KDIGO classification ≥ stage 3b
- • Acute kidney injury grade 3
- • Documented or suspected increased intracranial pressure (cerebral edema, brain compression, hydrocephalus, etc.).
- • Presence of mitral valve regurgitation \> grade 1
- • Presence of aortic valve regurgitation \> grade 1
- • Patients previously enrolled in the "COMPASS" study in the period of 12 preceding hours
