US-Guided PECS II Block Versus MTP Block for Postoperative Analgesia of MRM Sugery

This clinical trial is looking at two different ways to manage pain after a specific type of breast surgery called modified radical mastectomy (MRM). The goal is to see which method—either the modified pectoral plane block (PECS II) or the midpoint transverse process to pleura block (MTP)—is more effective at reducing pain in the first 24 hours after surgery. Researchers will measure pain levels using a simple scale that allows patients to rate their discomfort.
To participate in this study, women between the ages of 40 and 65 who are scheduled for MRM surgery may be eligible, as long as they are generally healthy according to specific medical guidelines. Unfortunately, individuals with certain health issues, those who are pregnant, or those who have had previous thoracic spine surgery cannot take part. If you join the trial, you can expect to receive one of the pain management techniques and help researchers understand which one works better for future patients.

Eligibility criteria
- Inclusion Criteria:
- • 1. Female patients scheduled for MRM.
- • 2. Age eligible ≥ 40 \& ≤ 65 yrs.
- • 3. Patients with American Society of Anesthesiologists (ASA) physical status I, II who will be scheduled for MRM surgery.
- Exclusion Criteria:
- • 1. Age \< 40 \& \> 65 years old.
- • 2. Declining to give written informed consent.
- • 3. History of allergy to the medications used in the study.
- • 4. Contraindication to regional anesthesia (including coagulopathy (platelet count ≤ 80,000, INR ≥ 1.5) and local infection).
- • 5. Severe hepatic impairment (INR ≥ 1.5, Bilirubin ≥ 2, Albumin ≤ 2).
- • 6. Renal dysfunction \[Glomerular filtration rate \< 50ml/min calculated by Modification of diet in renal disease equation for GFR estimation\].
- • 7. Psychiatric disorder.
- • 8. Pregnancy.
- • 9. Patient with history of thoracic spine surgery.
