Hot EMR vs Underwater Cold EMR for Large Colonic Adenomas

This clinical trial is comparing two methods of removing large growths from the colon called adenomas, which can sometimes lead to cancer. One method is called conventional endoscopic mucosal resection (EMR), and the other is a newer technique known as underwater cold EMR. The goal is to find out if the underwater cold method is just as effective and safe as the traditional method for removing these larger lesions.
To participate in the study, you need to be at least 18 years old and scheduled for a colonoscopy for reasons like screening or follow-up care. Participants will be individuals who can give their written consent and do not have certain types of lesions that might complicate the removal process. If you join the study, you will receive one of the two treatment methods, and the researchers will monitor your recovery and any potential recurrence of the adenomas. This trial is important as it could help improve the way these growths are treated in the future.

Eligibility criteria
- Inclusion criteria:
- • all patients ≥ 18 years of age undergoing colonoscopy for any indication (screening, anaemia, surveillance)
- • patients who were able to give informed written consent.
- Exclusion criteria:
- • lesions suspicious for submucosal invasion (e.g. Kudo V or Paris 0-IIa-IIc with nongranular surface).
- • lesions with large (\>10 mm) Paris 0-Is component that could compromise the nodular en-bloc resection and increase risk of submucosal invasion.
- • suspected sessile serrated adenomas (SSAs) according to traditional features such as adherent surface mucus, cloud-like surface, interruption of mucosal vessels, Kudo II-o pit pattern.
- • pedunculated polyps
- • active/quiescent colitis
- • patients with other lesions resected by hot snare during the same procedure.
- • rectal lesions
- • residual or recurrent adenoma after endoscopic mucosal resection
