This clinical trial is studying how well pharmacists in the Emergency Department at Methodist Charlton Medical Center can help resolve prescription problems for patients after they leave the hospital. The goal is to see how effective these pharmacists are in writing new prescriptions to fix issues that come up after a patient is discharged, ensuring they get the medications they need without delays.
To participate in this study, patients must have had a prescription written by an Emergency Medicine Pharmacist between November 2021 and June 2023. Participants can expect that their experience with the pharmacist’s intervention will be documented, helping researchers understand the impact of these actions on patient care. It’s important to note that certain patients, such as those who left the emergency department against medical advice or had issues unrelated to discharge prescriptions, will not be included in this study. Overall, this trial aims to improve the way prescription issues are handled, enhancing patient safety and satisfaction.

Eligibility criteria
- Inclusion Criteria:
- • Patients at MCMC who had a prescription sent by an EMP between November 1, 2021 and June 30, 2023 and a progress note and documenting the reason for the call and action taken by the EMP.
- Exclusion Criteria:
- • CPA actions were misclassified as discharge prescription issue resolutions.
- • Patients with prescriptions pursuant to late culture review, formulation changes not requiring a new prescription, prescription transfers to another pharmacy, or simple retransmission of prescriptions.
- • Patients documented to have left the ED against medical advice.
