The Arogya Sangama trial is a two-year project taking place in the Raichur and Chamarajanagar districts of Karnataka, India. The goal of this trial is to improve how people use health services and to enhance the quality of care provided at new Health and Wellness Centres (HWCs). By working closely with local government groups, known as Gram Panchayat Task Forces, the project aims to empower communities to take charge of their health services. To participate in this trial, you need to meet certain criteria. For women aged 18-49 who have given birth in the last year, and for individuals aged 30 and older who do not have diabetes or high blood pressure, you must live in the designated areas and speak Kannada. Participants can expect to be involved in activities that strengthen community health services and improve overall health knowledge and behaviors. This trial is important because it encourages local involvement in health care, aiming for better health outcomes for everyone in the community.

- • Group 1 Inclusion criteria
- • 18-49 years of age women who delivered in the last 12 months
- • Residents of the districts and living within the service área covered by intervention and control PHC-HWCs
- • Kannada speaking
- • Able to provide consent
- • Group 2 inclusion criteria
- • 30 or more years of age
- • Not a known case of diabetes and hypertension
- • Residents of the districts and living within the service area covered by intervention and control PHC-HWCs
- • Kannada speaking
- • Able to provide consent
- Exclusion Criteria:
- • Group 1: women not in the age group 18-49 years of age
- • Group 2: participants younger than 30 years
- • Not residing in the service area covered by intervention and control PHC-HWCs
- • Unable to speak in Kannada
- • Unable to provide consent
