This clinical trial is looking at a new way to help surgeons during surgery for a condition called primary hyperparathyroidism (PHPT), which is when the parathyroid glands produce too much hormone. The study is testing a special imaging technique called autofluorescence imaging (AF) to see if it can make the surgery safer and more effective. The trial is currently recruiting participants who are at least 18 years old and have been diagnosed with PHPT. To qualify, you should be planning to have a specific type of surgery called parathyroidectomy (PTX) that includes measuring certain hormone levels.

If you decide to participate, you will undergo the usual preparations for parathyroid surgery, and the doctors will use the new imaging method during the procedure. It's important to note that this trial is not for everyone; for example, if you have had previous thyroid or parathyroid surgery, or if you are planning to have other surgeries at the same time, you may not be eligible. Overall, this study aims to find out if this new imaging technique can help improve outcomes for patients undergoing surgery for PHPT.

Eligibility criteria

  • Inclusion Criteria:
  • Age\>18 years and able to give informed consent.
  • A diagnosis of PHPT.
  • Planned exploratory PTX with ioPTH measurement.
  • Exclusion Criteria:
  • Concordant high-confidence imaging; planned focused/selective PTX
  • Previous thyroid or parathyroid surgery.
  • Planned simultaneous thyroid surgery.
  • MEN, lithium induced hyperparathyroidism.
  • eGFR \< 30.
  • Kidney transplant.
