This clinical trial is looking at how a special worksheet and discussion with a pharmacist can help patients remember to take their new medications for heart disease or depression. The main goals are to see if patients find this approach helpful and easy to use, how well it helps them develop good medication-taking habits, and how it affects their beliefs and actions regarding taking their medications compared to those who only receive standard advice from a pharmacist.

To be eligible for the trial, participants need to be between the ages of 65 and 74, must be able to read and write in English, and should have been prescribed a new chronic medication. It's important to note that participants must be able to manage their own medications independently. If you join the trial, you can expect to fill out a worksheet and have a conversation with a pharmacist about your medication routine, which may help you build better habits for taking your medicines.
