This clinical trial is studying the removal of damaged first permanent molars in children aged 8 to 10 years. These teeth may be severely affected by conditions like molar incisor hypomineralization (MIH), irreversible pulpitis (which means the tooth is severely infected), or necrosis (where the tissue in the tooth is dead). By extracting these compromised teeth, the second permanent molars can move into position more easily as they grow in, helping to naturally close any gaps. This approach aims to provide a better long-term solution for dental health.
To be eligible for this study, children must be generally healthy and have specific dental conditions as mentioned, along with the presence of their second premolar teeth on X-rays. Parents will need to give their consent for their child to participate. If they join, families can expect to have the child’s tooth removed, which may help improve their overall dental alignment in the future. It’s important to note that children with certain dental issues or severe bite problems won’t be able to participate in this trial.

Eligibility criteria
- Inclusion Criteria:
- • Healthy patients\' class I according to the American Society of Anesthesiologists (ASA class I)
- • Age: 8-10 years
- • Angle class I occlusion
- • The radiographic presence of the second premolar
- • The radiographic stages of development E or F for formation the second permanent molar.
- • First permanent molar that is severely affected by MIH, signs of necrosis, signs of irreversible pulpitis or non-restorability.
- Exclusion Criteria:
- • Refusal of the parents to sign the informed consent.
- • Signs of reversible pulpitis of first permanent molar.
- • Patients with severe malocclusion.
