This clinical trial is studying the effects of adding a medication called dexmedetomidine to a common anesthetic, bupivacaine, during a specific type of pain relief procedure known as a bilateral erector spinae block (ESPB). This procedure is often used for patients undergoing lumbar fusion surgeries, which involve joining two or more bones in the lower back to relieve pain or improve stability. The goal of the study is to see if using both medications together provides better pain relief than using bupivacaine alone.

To be eligible for this trial, participants should be between the ages of 18 and 65, scheduled for lumbar spine fusion surgery, and in good overall health (classified as ASA physical status I-II, which means they are generally healthy or have mild medical conditions). Participants will receive either the combination treatment or just bupivacaine, and they will be monitored for their pain levels and recovery. It's important to know that individuals who regularly use strong painkillers, have certain medical conditions, or are unable to communicate effectively may not be able to participate. Eligibility criteria

  • Inclusion Criteria:
  • Patients of both genders
  • age between 18-65 years
  • American Society of Anesthesiologists (ASA) physical status I-II\*\*
  • scheduled for lumbar spine fusion surgeries ( 2 or 3 level lumbar fusion with or without decompression ) under general anesthesia .
  • Exclusion Criteria:
  • Patient refusal.
  • Patient with chronic use of opioid analgesia.
  • Uncooperative patients with communication difficulties, which might prevent a reliable postoperative assessment.
  • Contraindication to regional anesthesia (bleeding disorder, use of any anticoagulants, local infection).
  • Known allergy to local anesthetics.
