This clinical trial is focused on helping older adults who have previously experienced chronic homelessness to plan for their future healthcare needs. Many of these individuals are now living in permanent supportive housing, which provides them with stability. The trial aims to test new ways to ensure that these older adults can express their wishes about medical care, especially as they face health challenges and think about end-of-life decisions.
To participate, individuals need to be over 50 years old, have a history of chronic homelessness, and currently reside in permanent supportive housing. They must also be able to speak English. Unfortunately, those who have severe cognitive impairment, are deaf or blind, or cannot understand and provide consent will not be eligible. Participants in the trial will receive support in understanding and communicating their healthcare preferences. This is an important step in ensuring that their wishes are respected when it matters most.

- • Formerly chronically homeless adults living in permanent supportive housing
- • English speaking
- Exclusion Criteria:
- • Non-English Speaking
- • Screen positive for moderate-to-severe cognitive impairment
- • Self-report being deaf or blind
- • Cannot provide informed consent based on the teach back method
