This clinical trial is looking at two ways to handle the umbilical cord right after a preterm baby is born: umbilical cord milking and early cord clamping. The goal is to see if umbilical cord milking can help reduce the risk of death in non-vigorous preterm infants born between 30 and 34 weeks of pregnancy. Non-vigorous means that the baby may not be breathing or moving well at birth. The study is taking place at several hospitals and is currently recruiting participants.
To be eligible for this trial, infants must be born preterm, specifically between 30 weeks and just under 35 weeks of pregnancy, and must be non-vigorous at the time of birth. However, infants with certain conditions, like serious birth defects or problems with the umbilical cord, cannot participate. Families involved in this trial can expect to either have their baby's cord milked or clamped early after birth, and researchers will follow the babies' health during their hospital stay to see which method works better. This study is important because it aims to improve the care and outcomes for vulnerable preterm infants.

Eligibility criteria
- Inclusion Criteria:
- • Preterm infants born between 30 0/7 weeks to 34 6/7 weeks of gestation
- • Non-vigorous at birth
- Exclusion Criteria:
- • Infants with congenital malformation
- • Major chromosomal abnormalities
- • Complete placental abruption/cutting through the placenta at the time of delivery
- • Cord conditions (umbilical knots, inadequate cord length, cord rupture, non-reducible nuchal cord)
- • Mono-chorionic twins,
- • Twins with no information on amnion/chorion
- • Multiple gestation \>2
