This clinical trial is studying a special program designed to help stroke patients who have difficulty moving one side of their body, a condition known as hemiplegia. The goal of the program is to see how well it helps patients manage their own care, improves their quality of life, and reduces the chances of them needing to return to the hospital after they leave. The study involves 60 participants from Charoenkrung Pracharak Hospital, who will be divided into two groups: one will receive the transitional care program, while the other will not.
To participate, individuals must be at least 18 years old, have experienced their first stroke that affects movement, and have a moderate level of impairment. They should be able to read and communicate in Thai and must agree to take part in the research. Participants can expect to engage in the program while still in the hospital and continue it after they go home. This trial is currently looking for new participants, and it aims to offer valuable insights into improving care for stroke patients.

Eligibility criteria
- Inclusion Criteria:
- • Age from 18 years old.
- • The first diagnosis is acute ischemic stroke.
- • Hemiplegia
- • NIHSS 5-14 point (moderate impairment)
- • Telephone and Line Application used.
- • Can read and communicate in Thai language
- • Accept participants to research and inform consent.
- Exclusion Criteria:
- • Aphasia
- • The Six Item Cognitive Impairment Test: 6CIT from 8 points in age from 60 years old.
- • Participants with illness severity or dead during the study.
- • Reject the study
