The POPEYE trial is studying two different ways to manage pain after a specific type of surgery called open abdominal aortic aneurysm repair. This surgery can lead to significant pain and has some risks, so finding the best way to control pain is very important. The trial will compare a method called epidural analgesia, which uses a small tube placed in the back to deliver pain relief, with a newer technique called rectus sheath block, which involves injecting pain medication near the abdominal muscles. Researchers want to find out if the new method can provide similar pain relief while potentially reducing complications after surgery.
To participate in the trial, individuals must be adults over 18 years old who are scheduled for this type of surgery and have agreed to take part in the study. However, pregnant women, those with chronic pain, or people who have allergies to the medications used in the study cannot join. Participants will have the opportunity to contribute to important research that could improve pain management for future patients undergoing this surgery. It's a chance to potentially receive care that might help them feel better after their operation.

Eligibility criteria
- Inclusion Criteria:
- • adult patients (age \> 18 years)
- • patients who have given their consent to participate in the study
- • patients undergoing elective "open" surgical treatment of an abdominal aortic aneurysm with a midline xipho-pubic incision.
- Exclusion Criteria:
- • Pregnant women
- • Patients being treated for chronic pain
- • Known allergy to drugs included in analgesia protocols
- * Contraindications to the use of one of the two methods provided:
- • refusal by the patient
- • infection at the puncture site
- * coagulopathy:
- • 1. PTT \> 40 s and/or INR \> 1.4
- • 2. platelet count \< 50,000/µL
- • 3. taking antiplatelet or anticoagulant drugs that have not been discontinued in good time, according to international guidelines
- • Emergent intervention
- • Lack of consent to participate in the study
