ClinConnect Summary
The Aging Brain Care Virtual Program is a clinical trial designed to support individuals aged 65 and older who have dementia, including Alzheimer's disease. This program will help patients and their caregivers through regular virtual visits with specially trained nurses or social workers. For the first three months, caregivers will meet monthly, and then every few months depending on the needs of the patient. During these visits, the care team will assess the needs of both the patient and caregiver and provide tailored support to manage issues like behavioral symptoms of dementia, caregiver stress, medication management, and planning for future care.
To be eligible for this program, patients must be 65 or older, have visited their primary care doctor in the last 14 months, and have a diagnosis of Alzheimer's disease or related dementias. Caregivers who want to participate should be at least 18 years old, able to communicate in English, and have access to a phone or computer for virtual meetings. It's important to note that individuals living in a nursing home or those in hospice care cannot join the study. This program offers a unique opportunity for caregivers and patients to receive ongoing support in a convenient virtual format.
