This clinical trial is investigating the best way to treat a specific condition called spermatic cord lipoma, which can be found during a surgery called subinguinal varicocelectomy for men dealing with infertility due to varicoceles. The study aims to find out whether removing the lipoma (excision) or leaving it in place (preservation) leads to better sperm health, measured by semen quality, at 3 and 6 months after the surgery.
To participate in this trial, you need to be an adult male experiencing infertility and have a clinical varicocele, which is an enlargement of veins in the scrotum. You should also have a semen analysis showing low sperm count and poor sperm movement. However, if you are a younger male, have had a recurrent varicocele, have an inguinal hernia, or have no sperm at all (azoospermia), you would not be eligible for this study. If you qualify and decide to join, you will be assigned to one of the two groups and will have your sperm health monitored after the surgery to see which treatment works better.

- • Adult infertile males
- • Clinical varicocele
- • Semen analysis show oligo-athenospermia
- Exclusion Criteria:
- • Adolescent varicocele
- • Recurrent varicocele
- • Presence of inguinal hernia
- • Azospermic patients
