This clinical trial is studying different treatment options for patients with serious blockages in the blood vessels of the lower abdomen and pelvis, known as aortoiliac atherosclerosis. The goal is to compare three types of procedures: a minimally invasive technique called endovascular revascularization, a traditional surgery called aortobifemoral bypass, and a combination of both methods (hybrid approach). Researchers want to see which method works best for patients with advanced cases of this condition.
To participate in this trial, you need to be over 18 years old and have a specific type of blockage classified as TASC II type D. This means you have significant artery disease in the aortoiliac area that may require surgical treatment. If you join the study, you will receive one of the three treatments and have your health closely monitored to see how well the procedure works. It's important to note that if you have an aneurysm (a bulging area) in your aorta larger than 25 mm, you won’t be eligible for this trial. Overall, this study aims to improve options for patients with complex vascular issues.

Eligibility criteria
- Inclusion Criteria:
- • Patients with peripheral arterial/atherosclerotic disease of the aorto-iliac sector undergoing direct, hybrid or endovascular surgical correction with TransAtlantic Inter-Society Consensus (TASC II) type D classification(12).
- • Age \>18 years old
- Exclusion Criteria:
- • Synchronous aortoiliac aneurysmatic/ectasic disease (aorta AP diameter \>25 mm)
