This study tests whether three weight‑loss approaches can help adults with obesity improve insulin resistance, lose weight, and change body composition. Participants are randomly assigned to one of three groups: 1) intermittent fasting (eating within about 8 hours a day), 2) intermittent fasting plus behavioral economic rewards (money incentives for meeting weight‑loss goals or fasting targets), and 3) intensive lifestyle modification (structured guidance on healthy eating and exercise).
If you’re eligible (ages 18–65, BMI 25 or higher, and have not lost more than about 5% of your weight in the last 6 months), you would join the study for about a year. You’ll have blood and urine tests and body measurements at the start and at follow‑ups around 1, 2, 6, and 12 months, with additional checks on your diet, physical activity, and sleep. You’ll also complete brief questions about your activity and satiety and keep track of your food intake. The study takes place at Mahidol University in Bangkok, Thailand, and does not involve a drug or medical device.

- Inclusion Criteria:
- • Aged 18-65 year
- • BMI start at 25.0 kg/m2 (above 24.99 kg/m2)
- • Weight change less than 5% in 6-month
- • Ability and willingness to provide informed consent
- Exclusion Criteria:
- • Pregnant women or breed feeding
- • Kidney disease/ Liver disease
- • Abnormal Thyroid hormone, abnormal GI function
- • Postoperative bariatric surgery
- • Drug control weight or reduce appetite
- • Drug abuse
- • Mental illness
