This clinical trial is studying a new approach called the Rehabilitation Gaming System for Intensive Care Units (RGS-ICU), designed to help patients in critical care feel more comfortable and improve their mental health during and after their stay in the ICU. The goal is to see if this innovative program, which uses non-immersive virtual reality techniques, can provide early cognitive stimulation and psychological support to patients who are admitted to the ICU with serious conditions like respiratory failure or shock. Researchers believe that using the RGS-ICU alongside standard ICU care will improve patients' comfort during their hospital stay and their cognitive and mental health three and twelve months after they leave the ICU.

To participate in this trial, you must be an adult (18 years or older) admitted to a medical or surgical ICU for at least 48 hours and capable of communicating in Catalan or Spanish. Unfortunately, individuals with certain medical histories, such as neurological disorders or severe psychiatric illnesses, may not be eligible to join. Participants can expect to receive the standard care they would normally get in the ICU, along with the RGS-ICU intervention, which aims to enhance their recovery experience. This trial is important because it could lead to better care practices that improve both the quality of life for ICU survivors and reduce the long-term effects associated with critical illnesses.

Eligibility criteria

  • Inclusion Criteria:
  • Adult patients (≥18 years)
  • Admitted to a medical/surgical ICU
  • For respiratory failure, cardiogenic shock, or septic shock
  • With an expected ICU stay of ≥48 hours
  • Residing in Catalonia or Majorca (Balearic Islands)
  • Who speak Catalan and/or Spanish
  • Who are able to give informed consent by themselves or through an authorized representative
  • Exclusion Criteria:
  • History of intellectual disability or other neurodevelopmental disorders, such as autism spectrum disorder
  • History of neurological disorders, dementia or other neurodegenerative diseases, such as epilepsy, Alzheimer's disease, Parkinson's disease or multiple sclerosis
  • History of brain damage, such as traumatic brain injury or stroke
  • History of severe psychiatric illness, such as psychotic, bipolar, depressive, obsessive-compulsive, post-traumatic or personality disorder
  • Suspected or confirmed substance use disorder
  • Suspected or confirmed communicable disease in an isolated patient
  • Uncorrected hearing or visual impairment
  • Any medical condition that prevents safe upper extremity mobility, such as skin lesions, burns, or fractures
  • Life expectancy \<12 months
  • Enrolled in another trial that does not allow co-enrollment
