ClinConnect Summary
This clinical trial is studying how using patient-reported outcomes and experience measures (PROMs and PREMs) can improve health care for patients with breast cancer, prostate cancer, chronic kidney disease, or those undergoing bariatric surgery. Essentially, the trial aims to see if regularly sharing feedback about health and care experiences can lead to better health outcomes and increased satisfaction for both patients and health professionals.
To participate, you should be an adult patient receiving treatment for one of the specified conditions and comfortable using a smartphone or computer. If you join the study, you'll complete surveys each month using an app, which will help doctors understand your health better and improve communication.
