This clinical trial is looking at a new service called Medication Management Services (MMS) that helps patients better manage their medications, especially those with conditions like Type 2 diabetes and high blood pressure. The goal is to see if this service can improve how well patients stick to their medication routines, enhance their quality of life, and help them understand their health issues better. The trial will involve community pharmacies run by non-government organizations in Hong Kong, where pharmacists will guide patients in using their medications safely and effectively.
To participate, individuals need to be at least 18 years old, diagnosed with Type 2 diabetes and/or high blood pressure, and currently taking five or more medications, including at least one high-risk medication. They should also be able to communicate in Cantonese or English and must not have had any recent hospital admissions or changes in their medication. Participants will fill out questionnaires at various points during the trial to share their experiences and any changes they notice in their health or medication use. This study aims to understand how well this service works and whether it is cost-effective, ultimately benefiting patients and their caregivers.

Eligibility criteria
- Inclusion Criteria:
- • aged 18 years or above
- • diagnosed with Type 2 diabetes mellitus and/or hypertension
- • having regular follow-up on Type 2 diabetes mellitus and/or hypertension at Hospital Authority
- • no A\&E admission/hospitalization in the past 3 months
- • no recent changes in medication regimen in the past 3 months
- • polypharmacy (taking 5 or more chronic medications)
- • using at least 1 high-risk medication (Anticoagulants, Oral hypoglycaemics, Insulins, psychotropic medications or immunosuppressants)
- • able to communicate in Cantonese and/or English
- Exclusion Criteria:
- • aged below 18
- • unable to give informed consent
- • unable to communicate in Cantonese and/or English
