This clinical trial, titled "Supporting Resilience Among Re-entered Seniors," is designed to help older adults who have recently returned to their communities after being in jail or prison. To be eligible for this trial, participants must be at least 45 years old, have a chronic health condition, and have recently re-entered their community from incarceration. Those who join will first complete a survey and then take part in an 8-week program, meeting once a week, followed by three follow-up surveys to see how helpful the program was. This study is currently looking for participants, and it provides a chance for older adults to receive support tailored to their specific needs during this transition period.

- Inclusion Criteria:
- • at least 45 years old
- • have re-entered the community from a period of incarceration from jail or prison
- • have at least one chronic health condition
- • able to speak and understand English
- Exclusion Criteria:
- • aged 44 or younger
- • have not re-entered the community from a period of incarceration from jail or prison
- • does not have at least one chronic health condition
- • unable to speak and understand English
