This clinical trial is studying a special approach to care for people who have had a stroke. It's called the Transitional Care Model, which aims to help patients recover better after their hospital stay. The trial found that this model can lead to improvements in physical abilities, like being able to perform everyday tasks, along with fewer trips to the emergency room, reduced feelings of depression, and a better overall quality of life. However, researchers are still trying to figure out how effective this care model really is for different patients.
To participate in this trial, individuals need to be at least 18 years old and have been admitted to the neurology department for at least one day. They should also be able to understand and communicate in Indonesian and have good cognitive function, meaning they can think clearly and make decisions. Participants will be asked to take part during their hospital stay and for eight weeks after they leave. This study is currently recruiting participants and is open to everyone, regardless of gender. It's important to note that those with serious speech problems or certain psychiatric disorders cannot join the trial.

Eligibility criteria
- Inclusion Criteria:
- • Being at least 18 years old, being admitted in neurology department for at least 24 to 48 hours,
- • Not having cognitive impairment as measured by the Montreal cognitive assessment (score ≥ 23),
- • Being able to communicate in Indonesia, having the ability to provide informed consent,
- • Being willing to participate in this study during hospitalization and 8 weeks after hospital discharge
- Exclusion Criteria:
- • Have potentially biased information, such as being unable to provide informed consent due to cognitive impairment,
- • Having major speech and language problems or aphasia that prevent them from participating in the study,
- • Having serious psychiatric disorders or other with terminal disease requiring active treatment, and
- • Being discharged to a nursing home or welfare institution.
